What reasons for my appointments to be automatically canceled?

  • Incomplete mental health screening

  • Admissions documents incomplete

  • Confirmation of appointments

  • Tardiness to scheduled assessment more than 10 minutes.


How do we schedule an assessment?

Client & Parent / Guardian will complete free mental health screening. Once completed admissions will give you call and schedule the virtual assessment with a licensed clinician.

Who completes the Referral & Consent form?

Social workers, providers, counselors, and other healthcare professionals to ensure follow up care.

Do you accept all insurance?

Yes, in state and out of state: Medicaid, Medicare, and Commercial Insurance.

Do I have to pay our co - insurance, co - pay, and deductible at time of visit?

Yes, so we can properly update your insurance company.

How many clients attend group therapy Monday through Friday per a session?

A total of 12 clients and are separated by pediatric (ages 5 to12) and adolescent (ages 13 to 17).

Will a excuse note be provided for clients attending outpatient group therapy (PHP & IOP)? 

Yes, we will provide an excuse note for each day attended. Based on medical necessity.

What if I'm late pass 10 minutes to group therapy?

You will not be admitted into program and will be counted absent.


 

Physicians are on the medical staff of Haven Treatment Center, but, with limited exceptions, are independent practitioners who are not employees or agents of Haven Treatment Center. The facility shall not be liable for actions or treatments provided by physicians.  All physicians are contractors and bill for their services seperatley.
